Our offices are closed on holidays and the weekends. We are open Monday through Thursday form 8:00 AM to 4:45 PM and Fridays from 8:00AM to 12:30 PM.
In order to give the best possible care and attention to our patients and to protect their health of those who are pregnant, we are unable to allow children into our practice. If you need childcare during your appointment, please contact Kidz Bizz at 910-346-5551. Kidz Bizz is a drop-in childcare center located on 316 Dolphin Drive, Jacksonville.
Confidentiality and Medical Records
All your medical records are considered confidential information. We will release the records or any part of them only if you give us a written authorization. No matter what the circumstances, we will not release these records unless we have a written notification that you wish us to do so.
For your first appointment, it is recommended to download the following forms and complete them at home, bringing them to your appointment.

Insurance
We participate with Medicare, are PPO with Blue Shield, Medicaid and are a Carolina Access provider. We will file all insurance claims on your behalf regardless of our participation with the plan. If you are not sure about our provider status with your insurance company, please check with your insurance company or with our business office. Please be aware that co pays and deductibles are due at the time of your visit. If you have an Explanation of Benefits showing that you have met your plans deductible, please bring it with you to your appointment.
